Many medical insurance companies require written referrals from your primary care provider before they will cover services that cannot be provided in our office. With the exception of medical emergencies, the insurers usually require that the referral be done before the service is carried out. They may not approve referrals written "after-the-fact".

Please be aware that these referrals are based upon medical necessity. An appointment with your provider may first be required in order to evaluate whether the referral is needed. Virtually all referrals in our office are transmitted electronically, thus eliminating the need for our patients to make a separate trip to pick up referral forms. Please provide 24-48 hours advance notice for referrals whenever possible.
